City/Parish 3-1-1 Call Center is now providing online services for those customers who wish to submit service requests over the Internet. In addition to entering a service request, customers can follow the status and progress of their request online.

To submit a new request or to look up an existing service request please follow the instructions below:

Submit a New Request
Required Information: You will need the following information to start your Service Request.

Request Information: problem or request type, description of problem, and address or location where service is being requested.
Personal Information: name, address, and phone number of the person requesting service.

Click "Submit" to process your request.

Look up a Service Request
To check the status of an existing Service Request online you will need your Service Request Number.

Look up a Previously Submitted Service Request #
Click "Look up" to process your request.

PLEASE NOTE: If you entered in a request before August 9th 2004, you can not use your existing service request number to look up the status of your request. You must retrieve your new service request number with our Service Request Number Lookup Tool.
